  1. Print the registration form directly from CARAT (2nd window).

  2. Pay as per one of the following options:
    •   Pay by Fax - Write your Credit Card information, and the amount (see table below) on the registration form.
    •   Internet pay - Click on the appropriate pay option (in table below.) You will be redirected to a secured site, using the most up to date encryption mechanisms. Write the verification code on the registration form (in the space for the charge information.)

    •   Pay by Check - Write a check for the appropriate amount (see table below.)
  3. Complete the registration form and fax to +(845) 562-2699.

  4. Register CARAT (2nd window) using the unique registration number we provide for you (usually within the same business day.)
